Passports
 The
Township Clerk’s office is open daily, Monday thru Friday (excluding
township holidays and Furlough days) 8:00 AM to 2:00 PM for accepting
passport applications.
Each applicant must apply in person; minors must be present and
each applicant needs to have the following:
Proof of American citizenship (original birth
certificate, with raised seal, a previous U.S. passport (adults
only); a certificate of citizenship or naturalization certificate)
Valid photo I.D. (driver’s license, government I.D. or
military photo I.D.)
Two recent identical photos of applicant. The township is
not equipped to take passport photos at this time. You may bring
photos that meet passport requirements from another source, such as
CVS, Costco, AAA or the Post Office.
Both parents must be present when minor child is applying
or one parent and a notarized consent form must be presented for
children under the age of 16 years. One parent must also appear with
applicants over 16 years of age that do not have a valid photo I.D.
